Accidental tramline bruising and rib fractures

چکیده
linear implement and in children would be considered suggestive of maltreatment [1]. In this case it is hypothesised that the linearity was caused by compression of the skin against the tarmac by the ribs from within, rather than by compression by a rod from without. It is likely that such a mechanism requires a body mass substantially greater than the average child’s, so although this case demonstrates that linear bruising in an adult is not pathognomonic of a blow with a rod-like implement, in children it will raise suspicion of maltreatment.

PURPOSE Fractures are the second most common presentation of child abuse following soft-tissue bruising and burns. It is often difficult to determine potential abuse in a child presenting with a non-rib fracture(s) and without soft-tissue injuries. METHODS One hundred and fifteen consecutive patients aged ≤2 years who presented with a fracture between January 2010 and June 2012 to our emergen...
متن کاملPatterns of fractures in accidental and non-accidental injury in children: a comparative study.
The incidence and pattern of fractures in children who had been abused were compared with those of fractures sustained by children of similar ages in whom abuse had been excluded. From 1976 to 1982 there were 35 children with fractures resulting from child abuse, and all were aged under 5. Of the 826 children in the control group, seen from January to June 1981, 85% were aged over 5. Abused chi...

Non-accidental injury (NAI) in children includes orthopaedic trauma throughout the skeleton. Fractures with soft-tissue injuries constitute the majority of manifestations of physical abuse in children. Fracture and injury patterns vary with age and development, and NAI is intrinsically related to the mobility of the child. No fracture in isolation is pathognomonic of NAI, but specific abuse-rel...

Subjects, methods, and results I assessed 22 infants and children with fractures (mean age 6 months) who were the subjects of proceedings for child protection (19) or whose parents had been charged with causing their injuries (three). I scrutinised the medical notes, court papers, and x ray films for evidence of metabolic bone disease, particularly osteogenesis imperfecta (using the classificat...
